Notes:
¢ The waiting periods for basic,
major and orthodontic dental services shall be reduced by the number of
months for which each current employee was covered under the company's
prior comparable dental plan. Any waiting period listed under the Dental
Limitations and Exclusions can never be waived.
£ Platinum Plans
In-Network General Dentist & Specialist - All payments made by the
plan are based on the Platinum fee schedule.
In-Network General Dentist - Accept the Platinum fee schedule as payment
in full.
For services from all Out-of-Network Dentists, you are responsible for
charges exceeding the UCR.
¥ Specialists
Discount only - no benefit will be paid.
In-Network Secialist - You receive 20-25% off the specialist fee for covered
services.
You are responsible for the difference between the plan payment and the
discounted specialist's fee.
§ Endodontics
For Groups of 20 or more enrolled employees, Endodontics is a basic Benefit.
For Groups of 6-19 enrolled employees, Endodontics is a major benefit.
All Plans: The group's effective date is the first of
the month. Current employees and their dependents must enroll within 31
days of the group's first effective datre, and may not enroll again until
the group's annual renewal date or within 31 days of a qualifying event.
New emplyees have 31 days after their hire date or eligibility date to
enroll. Employees who cancel their plan may not enroll again until the
group's next renewal date.
